Physicians Surgical Hospitals aims to provide you with resources to help you determine your financial responsibility for services and procedures based on your insurance coverage and any qualifying discounts. The hospital bill does not include fees for your surgeon, anesthesia provider, radiologist, take-home equipment (DME) or pathology services. These services will be billed separately. While we make every effort to use providers who are in-network with your insurance plan, please be aware that our hospital does contract with outside physicians and services who may not be participating providers in your network.

I understand that my physician may be a partner in the ownership of Physicians Surgical Hospitals and I have the right to review a list of partners. The physicians and Allied Health Professionals (AHPs) practicing at Physicians Surgical Hospitals are licensed and/or credentialed to practice in this facility. The physicians and AHPs provide medical services at Physicians Surgical Hospitals, but they are not agents or employees of Physicians Surgical Hospitals.
