Physicians Surgical Hospitals aims to provide you with resources to help you determine your financial responsibility for services and procedures based on your insurance coverage and any qualifying discounts. The hospital bill does not include fees for your surgeon, anesthesia provider, radiologist, take-home equipment (DME) or pathology services. These services will be billed separately. While we make every effort to use providers who are in-network with your insurance plan, please be aware that our hospital does contract with outside physicians and services who may not be participating providers in your network.

What if I can’t pay my bill?
After your procedure, if you are unable to pay the remaining balance of your bill, please call 844-220-2194 to make payment arrangements or to discuss financial assistance opportunities.
Can I combine my open accounts?
If you have multiple accounts with Physicians Surgical Hospitals, you may be able to combine your accounts for more convenient payment arrangements.
In order to combine your accounts:
- All accounts must have current balances
- Any applicable insurance payments must be payed from all accounts
- No accounts can be with a collections agency
How can I request my medical records?
You can request a copy of your medical records by completing the Authorization to Disclose Patient Information Form.
You can submit this form in person at the main admitting area of Quail Creek Surgical hospital or by mailing it to the address below.

How can I get information concerning my Medicare coverage?

Physicians Surgical Hospitals meets the federal definition of a physician-owned hospital, according to 42 C.F.R. § 489.3. I understand that my physician may be a partner in the ownership of Physicians Surgical Hospitals and I have the right to review a list of partners. The physicians and Allied Health Professionals (AHPs) practicing at Physicians Surgical Hospitals are licensed and/or credentialed to practice in this facility. The physicians and AHPs provide medical services at Physicians Surgical Hospitals, but they are not agents or employees of Physicians Surgical Hospitals.
